We understand that we are all unique individuals and within our care services we need to know as much as possible about you to ensure that we can fulfil these needs. Each care package is specifically designed around your needs and preferences, once we have discussed your daily routine and listened to your requirements we will design a personalised care plan for you.

Care plans tailored for you

A home assessment visit will be arranged with one of our assessors to facilitate the planning of your care. Each individual care plan is put together in conference with those who are familiar with existing routines and needs. Care plans are arranged on your terms, and they are designed to meet your personal preferences, such as medication, meals, eating times, sleeping, and favourite activities. It will reflect any likes or dislikes you may have and will remain in your home. The final plan of care has to be consented to before any care commences. If your needs change, then our one-to-one care plan offers you the flexibility to change as your needs do.
